Reduce Aged Patient AR in Private Practices
In our recent posts we’ve touched on Old Patient AR. Today we’re focused on how to reduce it, with five practical tips designed to get those balances paid faster. If you’re new to medical billing, a quick refresher: accounts receivable is the money your practice is supposed to receive. Insurance AR comes from your fee schedule and contractual adjustments after services are billed. Patient AR is the portion actually owed by patients.
Defining what counts as “old” AR
Old AR typically means balances that are 60, 90, 120, or 180 days old and have had at least three statements sent. It can also apply to newer practices with a lot of new AR. The key is to identify what qualifies as old in your office so you can plan your next steps.
One: Tackle the Overdue Balances First - outreach and chart flags
Start by looking at each patient’s age and the amount they owe, ranking from oldest to newest or highest to lowest. For active patients, add a chart flag so you’re prompted to collect at the next visit. For inactive patients, a quick call or message can set expectations and outline next steps. If a balance is very old, it may be appropriate to involve collections after you’ve tried direct outreach. The goal is to maximize upfront collection so the balance doesn’t keep growing.
Two: Offer a “Paid By” Discount
When you send an invoice or statement, highlight a discount if the patient pays in full by a specified date—10% or 20% off by X date is a common and effective approach. This nudges timely payment and can reduce the number of accounts that go to collections. Just be sure your accounting reflects the discount so it doesn’t linger in old AR.
Three: Use Credit Cards on File
If you don’t already, collecting card details from eligible patients can be a proactive measure. You’ll need a signed financial responsibility agreement and informed consent, and you must store cards securely following PCI standards. Four: Collect What’s Due Before the Patient is Seen
This simple step can dramatically reduce post-visit balances. Make sure eligibility and insurance verification are solid, so you know the patient’s remaining deductible, copays, and coinsurance before they arrive. Some practices email pre-visit cost estimates or provide a payment link ahead of time, and you can collect any unpaid balances at check-in. A clear pre-visit payment policy can make a big difference.
Five: Time to Send to Collections
Last and typically least favorite is sending a patient to collections. This is never fun so it should be a last resort, the final option.
Use clear criteria: after at least three statements and once an AR age threshold (for example, 120 days) is reached, evaluate next steps. Inform patients upfront that nonpayment may lead to collections or a payment plan, and work with a reputable collection partner that understands patient privacy and state regulations. Remember, often the contract is with the insurer, not the patient, so you’re collecting patient responsibility within compliant guidelines.
